At SLUCare, we understand that your time is important, so we offer the following tips to assure your appointment goes as smoothly as possible.
If you are seeing a SLUCare doctor for the first time: Please reference your new patient packet. If you didn't receive a packet, please bring the following items with you to your appointment:
- Health insurance card and any required authorization or referral forms
- Driver's license or other photo identification and Social Security number
- Name and address of your referring doctor and primary care doctor
- Contact information for any other provider you would like us to communicate with regarding your care
- List of any medicines (and dosages) you are or have been taking (including over-the-counter medicines)
For faster check-in: Call SLUCare Central at 314-977-4440 to verify or update your account before your visit. This will help our registration staff to assure your eligibility and benefits for services prior to your appointment.
At the registration desk: If you will be checking in at the registration desk, please arrive 15 minutes before your scheduled appointment time. We will verbally verify your information and collect your co-payment. You will also be given the opportunity to remit payment for your bill.
Now Available: Online Patient Pre-Registration
In our continuing effort to deliver a streamlined patient experience, SLUCare is now using an online service that allows you to pre-register for your doctor visit before you come to the office. Using your smartphone, tablet or computer, you can access the secure portal to update your information at your convenience. There is no special software or app required.
Here’s How It Works
You will receive an email or text from SLUCare Physician Group with a reminder about your appointment and a link to pre-register. You cannot pre-register online without this unique link.
When you click the link, you’ll be asked to confirm your identity with a phone number and date of birth.
Once confirmed, you will be led through a series of screens that may include:
- Travel screening questions
- Your contact information
- Your doctor’s name
- Other personal information, such as employment and marital status
- Contact information for the person responsible for your medical bill
- Emergency contact details
- Insurance information
- Consent for treatment
- Copay information
- Menu of payment options
- Directions to your appointment
You will be given the option to confirm or update the information we have on file for you. You will also be asked to upload images of your insurance card(s) and photo ID/driver’s license.
By completing your registration ahead of your appointment, you’ll save time and limit discussion of your personal information at the registration desk when you arrive.

At the doctor’s office: If you’ve completed your pre-registration before your arrival, just check in at the front desk to let us know you’ve arrived, so we can call you into a room at the appropriate time.
If you’ve pre-registered online: You may sign in at the kiosk when you arrive at your doctor’s office instead of checking in at the registration desk. This lets us know you’ve arrived, so we can call you at the appropriate time for your appointment.
